  • My practice is in network with all Blue Cross Blue Shield PPO plans.

  • I accept Auto Insurance, Workers' Compensation, and JW HealthCare Support.

  • I accept out-of-network insurance on a case-by-case basis*

  • I submit claims to all of the above directly.

  • I am not a Medicare provider, and they will not reimburse for treatment at my practice.

  • Please consider booking Health & Wellness appointments if insurance is not an option.

​*Note: If you have a commercial health insurance that is not listed above (Aetna, Cigna, Humana, United Health Care, etc), I would be considered an "out-of-network" provider. In this case, you would be responsible for any difference between my current Health & Wellness rate and what your insurances reimburses me.



Whether in-network or out-of-network, if you would like to know what your financial responsibility will be before you begin care, please contact me, and provide your health insurance Member ID and your date of birth.

My concierge practice also offers Health & Wellness appointments. Accepted forms of payment include cash, check, credit, debit, HSA card, FSA card, and Zelle, and the rates are displayed in the booking widget on the Appointments page.
