• My practice is in-network with all Blue Cross Blue Shield PPO plans.

  • I accept Auto Insurance.

  • I accept Workers' Compensation.

  • I accept JW HealthCare Support.

  • I submit claims to all of the above directly.

  • I am not a Medicare provider, and they will not reimburse for treatment at my practice.

Note: If you have a different commercial health insurance (Aetna, Cigna, Humana, United Health Care, etc), I would be considered an "out-of-network" provider. But, I may be able to honor your in-network benefits. In this case, your financial responsibility would be the same as it would if you were to attend an in-network provider. Please contact your insurance to request your "financial responsibility for out-of-network outpatient physical therapy", and then reach out to me directly to discuss this option. Likewise, if you would like to know what your financial responsibility will be with your in-network benefits before you begin care, please contact me directly.
